LEVERAGING INTUS CARE ANALYTICS TO INNOVATE FALL PROGRAMS IN PACE
PACE of the Southern Piedmont located in Charlotte, North Carolina serves local seniors in Cabarrus, Mecklenburg, Union, and Stanly counties. The Program of All-Inclusive Care (PACE), which opened in 2013, provides interdisciplinary services to clinically complex participants, allowing them to remain in their homes while receiving comprehensive and quality healthcare services.
APPLYING INTEGRATED CARE SERVICES TO IMPROVE AND INNOVATE PACE
Intus Care has provided us a practical, rather than theoretical, roadmap to success which is far more effective to achieve better health outcomes for our participants.
Midland Care PACE began working with Intus Care in 2022. As the organization entered 2023, they wanted to reduce unnecessary utilizations among their participant population. The program worked with Intus Care’s Integrated Care Services (ICS) team to review data from the platform and uncover strategies to lower utilization.
In 2007, Midland Care became the nation’s 37th Program of All-inclusive Care for the Elderly (PACE), a national program providing community-based care and services to those who would otherwise need nursing home levels of care. The program served 27 participants in its first year and has grown to serve over 550 participants across twelve Kansas communities.
LIFE-NWPA CASE STUDY: IMPACT OF DATA AND ANALYTICS FOR A FALLS REDUCTION INITIATIVE
Living Independence for the Elderly – Northwestern Pennsylvania (LIFE-NWPA) helps individuals with chronic care needs to be served in their homes and communities through an all-inclusive and preventative approach to care. LIFE-NWPA currently serves 650 participants in 13 Pennsylvania counties through an innovative and effective care team.
