Intus Care, Inc. (“IntusCare”) is committed to protecting your privacy. his Privacy Policy explains how we collect, use, disclose, and safeguard your information when you interact with us online.

This Privacy Policy applies to the information that we collect from or about you when you:

• visit our website https://intuscare.com/ (the “Website”); or
• access, use, or otherwise interact with our cloud-based software solutions (collectively, the “Solutions”) as an administrator or authorized user of an IntusCare customer.

This Privacy Policy refers to the Website and our Solutions collectively as the “Services.”

This Privacy Policy does not apply to information we collect offline or through any other products or services not described herein.

This Privacy Policy also does not apply to information and data that is created or displayed within, or processed by our customers through, our Solutions, such as health information about participants in our customers’ PACE programs. IntusCare processes that information on our customers’ behalf, as a “business associate” of those customers, and pursuant to our agreements with them. If you have questions about the collection and handling of your personal information by a customer of ours through their use of our Solutions, please contact that customer directly.

The Information We Collect and How We Collect It.

The information we collect, and how we use and disclose it, depends on the specific features of the Services you use and on the context of your interactions with us. As explained below, we collect some of this personal information when you provide it directly to us, or when we receive it from third parties. We also collect certain information automatically when you use and interact with the Services.

Information You Provide Directly

When you use the Services, or contact us by phone or email, we will collect personal information that you choose to share with us, which may include information falling within the following categories:

• personal and contact details such as your name, mailing address, email address, and phone number;
• account and profile information, such as your username and password, that is used to access the Solutions;
• business information such as your company name and job title/position;
• payment and billing information; and
• information that you choose to share when you fill out and submit contact forms that we make available through the Services, or otherwise communicate with us through the Services.

Information We Receive from Third Parties

We may also collect information about you from other sources, including third-party data providers, social media platforms, and publicly available information, and combine this information with information that you provide directly. The information we collect from other sources may include your name and email address, and professional details such as your employer and title/position.

By using the Services, you acknowledge that we may transfer your information to jurisdictions that may have different data protection rules than in your country.

• You may choose not to provide the information we request; however, this may limit your ability to use the Services, or certain features of the Services.
