This month’s CareHub EMR release is designed around this principle, reducing the cognitive burden on care teams by automating routine tasks, surfacing issues sooner, and helping staff move care forward without relying on manual follow-up.

Connect assessments to action

Care moves fastest when the next step is already in motion before the current one is finished. CareHub removes one of the most common workflow interruptions by connecting assessments directly to scheduling. Staff can now create a scheduling request or book an appointment directly from the assessment task, with participant information carried over automatically. Once the appointment is created, the task updates to show it has been scheduled, with a direct link to the event.

The result is a continuous workflow instead of a handoff. Because assessments, referrals, and appointments stay connected in CareHub, the IDT can easily see what has already been scheduled and what still requires attention.

This release also expands the assessment experience with three additional clinically validated tools all available directly in CareHub:

  2. FAST Score for Dementia
  3. 30-Second Chair Rise Assessment

Prevent scheduling surprises

Many scheduling issues aren’t caused by a lack of available appointments. They happen because the supporting resources (e.g., transportation and on-site aides) are limited and shared across participants. Often, teams don’t discover these conflicts until the schedule is already in motion.

CareHub’s new Current Resource Use panel helps teams prevent these conflicts earlier. Now teams can click on this panel from the event details and see the specifics on all overlapping visits, including date, time, event type, transportation and on-site aide requirements.

Additional scheduling enhancements in CareHub reduce the amount of data entry for PACE teams. Transportation events now capture pickup and drop-off locations that automatically flow into Scheduling Report exports. On-site aide requirements from CareHub participant profiles automatically apply to new events, eliminating another manual step during scheduling.

Together, these updates help schedulers spend less time managing logistics and more time ensuring participants receive services as planned.

Keep enrollment current

Enrollment forms often require corrections before enrollment is complete. For California PACE organizations, CareHub now allows users to edit and print the DHCS 4241 PDF (Level of Care) during any open enrollment stage, ensuring form details can be updated as soon as information changes.

Find the right participant faster

In busy PACE organizations, it’s common for multiple participants to share the same name. Staff need a quick, reliable way to confirm they’re working in the correct record.

Participant selection menus throughout CareHub now display each participant’s Member ID alongside their name. This added identifier helps teams quickly distinguish between participants with similar names and select the correct record with confidence.

Protect sensitive grievance information

Grievances require visibility, but not every user needs access to every grievance. CareHub now gives organizations more control over who can view and manage grievance information through a dedicated permission structure within a security role.

Administrative users retain full visibility, while non-administrative users are limited to grievances they created or were assigned. Because grievance tasks inherit the same permissions automatically, programs can maintain consistent access controls without extra configuration.
