CareHub now supports configuring assessment content and structure to reflect how PACE teams actually operate. Programs can adjust questions and flows directly in the platform. You can shape sections, assign responsibility by discipline, and determine where content belongs across different assessment types. This brings assessments closer in line with your workflows instead of static templates.

CareHub also ensures assessments remain connected to downstream work. As timelines change, related discipline tasks stay aligned, helping teams move from assessment to action without manual coordination or follow-up.

This month’s updates continue to improve alignment between Scheduling and Service Tracking, so appointments, providers, and service status updates behave consistently across workflows. Coordinators can now access full scheduling functionality directly from a service request, ensuring authorized services are booked accurately without switching contexts.
